About Insurance
WHAT HEALTH INSURANCE DO YOU ACCEPT?
Serenity Holistic Health does not bill any insurance directly. Clients are responsible for payment in full at the time of their appointment. Serenity Holistic Health will provide a superbill as a courtesy to our patients upon their request. A superbill is an enhanced invoice completed by your treatment practitioner containing insurance billing codes that allows you to create a claim to submit to your insurance company for possible reimbursement. A superbill does not guarantee your insurance will cover the cost of your visit. Please notify the practitioner at the beginning of your appointment, or admin at check-in, if you would like to have a superbill created for your visit. We always recommend that you contact your insurance company to find out what your policy will reimburse and at what rate ahead of time.

HOW DO I KNOW IF MY INSURANCE WILL COVER MY TREATMENT?

Call the customer service number listed on your insurance card to enquire what coverage you have under your policy.

Important information to ask your insurance company's customer service:

Start by letting them know: “I want to work with an out-of-network licensed medical professional, how much will you reimburse me?”​

Remember to mention (1) the specific plan you have with the insurance company and, (2) services you plan to seek with us (i.e. acupuncture, annual preventative visits, dietary counseling, therapy for chronic pain, etc.)

Naturopathic Doctor appointments only:

​Some insurance company customer service representatives may not be aware of Naturopathic Medicine. If they ask for clarification, you can tell them that all of our Naturopathic Doctors are also Chiropractic Physicians who are licensed healthcare providers in the state of Illinois. They are trained as primary care providers and provide preventative healthcare that fulfill medical care standards established by the medical community.​

SUBMITTING YOUR SUPERBILL FOR REIMBURSEMENT
We recommend submitting the following information to your insurance company as part of the claim:​

Superbill from your appointment

Photocopy of your insurance card, front & back

Any other documentation your insurance company may have requested
Remember to keep a digital copy of all documents you submitted for your own records!

RELEASE AUTHORIZATION
Be aware that if you choose to submit a superbill, you are releasing medical information that is protected by law. This means you are waiving some of your rights to privacy and confidentiality. It is standard for your insurance company to keep a record of your diagnoses stated on the superbill as part of your permanent medical file.
